Job summary

Palestine Regional Medical Center in Palestine, TX, is seeking a dedicated Social Worker to coordinate and execute social work functions across the care continuum in collaboration with the Interdisciplinary Care Transitions team.

You will assess psychosocial needs, advocate for patients and families, arrange discharge planning and post-hospital referrals, educate on patient rights and resources, and participate in rounds to ensure seamless, patient-centered care.

Qualifications

• MSW preferred; graduate of an accredited program.
• Must read, write and speak fluent English.
• Experience in healthcare setting preferred.
• Knowledge of discharge planning and community resources helpful.

Responsibilities

• Coordinate discharge planning with the Interdisciplinary Team and physicians.
• Conduct comprehensive biopsychosocial assessments.
• Advocate for patients and families; educate on post-hospital needs.
• Participate in rounds and provide case management.
• Make referrals to community resources to support care after discharge.
