Holy Cross Health in Germantown, MD, is seeking a part-time Social Worker to join our Care Coordination team on the Day shift. The role focuses on assessment, planning, facilitation, and advocacy to meet patients’ healthcare needs.

You will help identify resources, promote quality outcomes, and contribute to cost-effective care under the Manager of Care Coordination. The position requires an MSW, at least one year of acute-care experience, and Maryland LGSW/LCSW/LCSW-C licensure.

This position reports to the Manager of Care Coordination.

As a “Social Worker” You Will

• Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc.
• Collaborates with the Care Coordination staff and multidisciplinary team members regarding discharge planning and arrange for needed care in the home or an alternate setting.
• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.
• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.
• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
• Ability to perform diversified duties with time limitations with a high degree of accuracy
• Must be able to work independently and on a team.
• Demonstrates ability to use problem solving, critical thinking and priority setting skills

Minimum Qualifications

• Master's Degree in Social Work/Services
• At least one year of social work experience in an acute-care setting (MSW internship experience is acceptable).
• State of Maryland LGSW, LCSW or LCSW-C licensure.
• Demonstrated ability to work with all age groups of patients.
• Able to communicate effectively, demonstrating good audio and visual acuity.
• Ability to prioritize tasks and manage time effectively.
• Reports to and is evaluated by the Director of Case Management and the Manager.

Pay Range: $34.07 – $52.84

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Non-benefit eligible Roles

• Under 0.3 FTE (16 hours weekly) or per diem roles**
