Job summary

Holy Cross Health in Germantown, MD, seeks a part-time Social Worker to collaborate with Care Coordination staff in assessment, planning, facilitation, and advocacy to meet patients' health care needs.

You will support discharge planning, provide counseling to patients and families, document interventions, and participate in multidisciplinary rounds, contributing to quality outcomes and cost-effective care.

Qualifications

• Master's Degree in Social Work/Services is required.
• At least one year of social work experience in an acute-care setting (MSW internship acceptable).
• Maryland LGSW, LCSW or LCSW-C licensure is required.
• Demonstrated ability to work with all age groups of patients.
• Able to communicate effectively with good audio/visual acuity.
• Ability to prioritize tasks and manage time effectively.

Responsibilities

• Collaborates with Care Coordination staff and multidisciplinary team regarding discharge planning and arrange for needed care in the home or an alternate setting.
• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.
• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.
• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
• Must be able to work independently and on a team.
• Demonstrates ability to use problem solving, critical thinking and priority setting skills.

Position Purpose

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient's health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery. This position reports to the Manager of Care Coordination.

Responsibilities

As a 'Social Worker' you will:

• Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc.
• Collaborates with the Care Coordination staff and multidisciplinary team members regarding discharge planning and arrange for needed care in the home or an alternate setting.
• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.
• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.
• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
• Ability to perform diversified duties with time limitations with a high degree of accuracy
• Must be able to work independently and on a team.

Minimum Qualifications

• Master's Degree in Social Work/Services
• At least one year of social work experience in an acute-care setting (MSW internship experience is acceptable).
• State of Maryland LGSW, LCSW or LCSW-C licensure.

• Able to communicate effectively, demonstrating good audio and visual acuity.

Reports to and is evaluated by the Director of Case Management and the Manager.
