Job summary

Lutheran SeniorLife in Zelienople, PA seeks a full-time Social Worker for a daylight shift. You will advocate for residents, develop psycho-social care plans, and support families as part of an interdisciplinary team.

Salary starts at $21.40/hr; a $4,000 sign-on bonus is paid throughout the first year. Requires a Social Work license or equivalent degree and at least 1 year in health-related field.

Qualifications

• Current license under the Social Workers, Marriage and Family Therapists and Professional Counselors Act, or a degree in social work/social welfare.
• 1 year experience in health or related field.
• Strong written and verbal communications skills.
• Working knowledge of computers, Microsoft Word and Excel required.

Responsibilities

• Advocate for residents in a person-centered culture; identify and develop care plans addressing psycho-social needs.
• Ensure appropriate care plans for assigned residents/patients; act as advocate; provide staff education as appropriate.
• Develop goals with residents/patients and collaborate to achieve them; liaison to residents and families; provide educational support.
