Job summary

Lutheran SeniorLife is seeking a Social Worker in Zelienople, PA to advocate for residents within a person-centered culture. You will develop care plans addressing psycho-social needs and collaborate with an interdisciplinary team to support residents and families.

You will identify goals with residents and provide education to families and staff as needed, contributing to a compassionate, supportive work environment in western Pennsylvania.

Qualifications

• Current license or degree in social work field as specified.
• 1 year experience in health or related field.
• Strong written and verbal communications skills.
• Working knowledge of computers, Microsoft Word and Excel required.

Responsibilities

• Advocate for residents in a person-centered care culture.
• Develop care plans addressing psycho-social needs of residents/patients.
• Act as liaison and provide educational support to residents and their families.
• Identify and develop goals for residents/patients and help achieve them.
• Educate staff as appropriate on care plans and resident needs.
