Job summary

Healthcare Nursing of Elizabeth is seeking a Per Diem CSW Social Worker to join our team in Union County, NJ. You will handle admissions, assessments, care planning, MDS 3.0 tasks, Medicaid applications, advocacy, crisis intervention, and discharge planning for our sub-acute and long-term care residents.

The ideal candidate holds a NJ CSW License in good standing, demonstrates strong writing and communication skills, and works well in a team.

Qualifications

• Must have a NJ Social Work Certification CSW License in good standing.
• Must be able to multi-task and organize.

Per Diem CSW Social Worker for a sub-acute and long-term care facility located in Union County, NJ to join our team of professionals.

Duties include, but are not limited to:

• Admission and Ongoing Assessments/Documentation
• Helping with Adjustment
• MDS 3.0
• Care Planning
• Assisting with Medicaid Applications and other Financial Matters
• Advocacy
• Crisis Intervention
• Discharge Planning

To qualify for this position, the ideal Candidate:
