Qualifications

• NJ LSW/CSW/LCSW licensure required.
• 1–2 years in healthcare (SNF/long-term care or rehab) preferred.
• Experience with discharge planning, Medicaid/Medicare, MLTS preferred.
• Understanding CMS regulations and Department of Health survey process preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provide emotional support and advocacy for residents and families.
• Participate in interdisciplinary care plan meetings.
• Educate residents and families regarding Advanced Directives.
