Job summary

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program.
• Licensure as required for practice in the state.
• Experience with medical/behavioral health integration encouraged.

Responsibilities

• Conducts comprehensive biopsychosocial and mental health assessments.
• Responds to mental health crises in clinical and community settings.
• Provides clinical case management addressing medical, behavioral, social, and cultural determinants of health.
• Provides consultation to staff on psychosocial and behavioral health factors affecting patient care.
