Qualifications

• Bachelor's or MSW from an accredited institution.
• Current state licensure as LSW or RSW depending on state.
• Minimum of one year of social work experience preferred.
• Complies with professional standards of practice.
• CPR certification if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

The Social Worker - Clinical Review Team Lead collaborates with themulti-disciplinary team to provide input in the development of the plan of carefor those clients requiring social work intervention. The Social Worker - Clinical Review Team Lead will providesupport, advocacy, and resources to individuals and families facing variouschallenges. This role involves conducting assessments, developing care plans,coordinating services, and collaborating with interdisciplinary teams topromote overall well-being.

Minimum Requirements:

• Bachelors or Master’s Degree in Social Work (MSW) from anaccredited institution required

• Current state licensure as a Social Worker - Clinical Review Team Lead (LSW)required or current state registration as a Registered Social Worker (RSW)depending on state in which they will be working.

• Minimum of one year of social work experience preferred
