Summary:

The Social Worker at CHANS Home Health & Hospice works as part of the interdisciplinary team to provide individuals, families, and caregivers with psychosocial support to cope with chronic, acute, or terminal illnesses. Services include advising patients and family caregivers about community resources and supports, and guiding individuals through the process as needed.

Required Minimum Knowledge, Skills, and Abilities (KSAs):

1. Education: Bachelor's Degree in Social Work required.
2. License/Certifications: Current Maine licensure as an LSW required.
