Responsibilities

• Assesses all patients referred for social work case management services.
• Prescreening for high risk patients via census review and rounds.
• Collaborates with interdisciplinary team to develop/organize treatment plans with optimal outcomes.
• Coordinates plans with patient/family and health care team.
• Provides supportive counseling and crisis intervention.
• Maintains required documentation and complies with state/federal mandates.
