Qualifications

• MSW degree required.
• LICSW licensure required.
• Minimum four years of social work experience in healthcare, psychiatric and substance use settings.

Responsibilities

• Provide psychosocial, risk and level of care assessments for high-risk patients.
• Complete initial social work assessments at admission for patients assigned to Social Work.
• Deliver brief therapeutic interventions with patients and families.
• Develop discharge plans with patient, family and interdisciplinary team to ensure safe disposition.
