Qualifications

• Bachelor’s degree in social work or related field required; master’s degree strongly preferred.
• Minimum of one year of social work or case management experience in a healthcare setting.
• Knowledge of regulations, policies, and guidelines governing skilled nursing facilities.
• Strong communication, organization, and problem-solving skills.
• Ability to interact tactfully with residents, families, staff, and external agencies.

Responsibilities

• Assess residents’ psychosocial needs, strengths, goals, and barriers to care.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and resource referrals to residents and families.
• Advocate for residents, ensuring their needs, preferences, and concerns are communicated and respected.
• Maintain communication among residents, families, physicians, clinical departments, and external providers.
• Participate in care planning meetings and monitor progress as residents’ needs change.
• Coordinate discharge planning, referrals, appointments, transportation, and community services to support continuity of care.
• Educate residents and families about available services, care plans, and transition options.
• Maintain accurate documentation and comply with facility policies, regulations, and professional standards.
