Chestnut Woods Rehabilitation and Healthcare Center is hiring a Social Worker Case Manager to support residents and families through individualized care planning, advocacy, and resource access during stay and transitions.

The role emphasizes psychosocial assessment, care coordination, discharge planning, and education, with a focus on compassionate, compliant, and person-centered care in a sub-acute and long-term care setting.

Qualifications

• Bachelor’s degree in social work or related field required; master’s degree strongly preferred.
• Minimum of one year of social work or case management experience in a healthcare setting.
• Knowledge of regulations, policies, and guidelines governing skilled nursing facilities.
• Strong communication, organization, and problem-solving skills.
• Ability to interact tactfully with residents, families, staff, and external agencies.

Responsibilities

• Assess residents’ psychosocial needs, strengths, goals, and barriers to care.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and resource referrals to residents and families.
• Advocate for residents, ensuring their needs, preferences, and concerns are communicated and respected.
• Maintain communication among residents, families, physicians, clinical departments, and external providers.
• Participate in care planning meetings and monitor progress as residents’ needs change.
• Coordinate discharge planning, referrals, appointments, transportation, and community services to support continuity of care.
• Educate residents and families about available services, care plans, and transition options.
• Maintain accurate documentation and comply with facility policies, regulations, and professional standards.
