Inova Mount Vernon Hospital - Acute Inpatient Rehabilitation Unit is seeking a dedicated Registered Nurse Case Manager 2 to join our team.

Inova Mount Vernon Hospital is a 237-bed facility providing state-of-the-art care in a community setting. Our hospital is situated on 26 acres of landscaped open space, featuring gardens for patient serenity.

We are home to the nationally recognized Inova Joint Replacement Center and Inova Rehabilitation Center, ranked among the top orthopedic and rehabilitation programs in the country. Additionally, our Inova Wound Healing Center is the region's largest and most experienced.

The hospital recently opened the Veatch Family Emergency Department, a modern facility with 35 private treatment rooms, fast track areas, advanced ambulance access, cardiac emergency rooms, and a pediatric treatment zone.

Responsibilities of the Registered Nurse Case Manager 2:

1. Develop, implement, and evaluate patient care plans throughout the care continuum.
2. Collaborate with physicians, nurses, and the multidisciplinary team for effective patient management.
3. Provide discharge planning and care continuity in acute and post-acute settings.
4. Coordinate services and serve as a liaison between patients, families, and healthcare team members.
5. Utilize utilization management techniques to assess the necessity and appropriateness of healthcare services.

Requirements:

Work Schedule: Monday - Friday, 8:00 am - 4:30 pm, with every 5th weekend and one holiday per year.

Education: Bachelor's Degree in Nursing or Associate's Degree. ASN candidates must obtain a BSN within 5 years of start date.

Experience: At least two years in clinical care or case management.

Licensure: Active RN license in Virginia or multi-state license.

Certification: Accredited Case Manager, CCM, or Care Guidelines Specialist certification through MCG.

Additional Certification: BLS certification from the American Heart Association.

Preferred Qualifications:

Experience working with physicians in a hospital setting is preferred.
