Job Opportunity: Social Worker - Discharge Planning

This position is eligible for a $9,000 sign-on bonus!

Job Summary:

Responsible for collaborating with healthcare team members to coordinate appropriate, cost-effective services for members, helping them achieve optimal independence and quality of life.

Essential Responsibilities:

  1. Conduct problem identification, psychosocial assessments, financial counseling/referrals, and access to community resources.
  2. Manage and coordinate caseloads effectively.
  3. Complete comprehensive psychosocial assessments to evaluate patient goals, support systems, health status, and response to treatment.
  4. Develop and monitor care plans in collaboration with the healthcare team.
  5. Identify and facilitate access to organizational and community resources, ensuring timely and cost-effective implementation.
  6. Maintain accurate documentation of case management activities.
  7. Analyze outcomes to improve program quality and cost efficiency.
  8. Assist with applications for programs like Medicaid, Medicare, and Disability, and report suspected abuse/neglect.
  9. Provide counseling and education to members, families, caregivers, and healthcare team members, including end-of-life issues and Advanced Directives.
  10. Stay informed on current case management standards and community healthcare innovations.
  11. Maintain knowledge of relevant laws, regulations, and policies, adhering to NASW standards.
  12. Coordinate family meetings and assist with guardianship paperwork and virtual court attendance as needed.
  13. For inpatient roles, complete UAIS forms for long-term care (Virginia Medicaid).
  14. Perform other duties as assigned.

Additional Details:

Work in the ED with every other weekend and holiday requirements.

Qualifications:

  • Bachelor's degree in social work required.
  • Credentials from multiple states required; primary state credentials needed before hire, additional credentials post-hire.
  • Licenses required within 6 months of hire: Maryland Licensed Bachelor Social Worker, District of Columbia Licensed Social Work Associate, Virginia Licensed Baccalaureate Social Worker.

Preferred Skills:

  • Experience with Windows-based software.
  • Knowledge of community systems/resources in the service area.
  • Understanding of regulatory issues in the Mid-Atlantic region.

Work Details:

  • Location: Virginia, Arlington, Virginia Hospital Center
  • Part-time, 24 hours/week, Day shift (Week 1: Wed/Sat; Week 2: Sun/Tue), 7:00 AM - 7:30 PM
  • Employee Status: Regular, Union: M38 UFCW Local 400, Entry Level
