We are seeking a dedicated Social Worker Case Manager 2 to join Inova Mount Vernon Hospital, specifically within our Acute Inpatient Rehabilitation Unit. This hospital is a 237-bed facility providing state-of-the-art care in a community setting, featuring beautifully landscaped gardens and specialized units such as the Inova Joint Replacement Center and Inova Rehabilitation Center.

Key Responsibilities:

• Develop, implement, and evaluate patient care plans
• Collaborate with healthcare team members for patient management
• Coordinate discharge planning and continuity of care
• Serve as a liaison among patients, families, and healthcare providers
• Utilize utilization management techniques for healthcare services

Minimum Requirements:

• Monday - Friday, 8:00am - 4:30pm schedule, with every 5th weekend and one holiday annually
• Bachelor's Degree in Nursing or Associate's Degree (with BSN within 5 years)
• Minimum of two years clinical care or case management experience
• Active RN license in Virginia or multi-state license
• Accredited Case Manager certification or equivalent
• Basic Life Support certification from AHA

Preferred Qualifications:

• Experience working with physicians in a hospital setting
