Job summary

Manatee Memorial Hospital in Bradenton, FL seeks a nurse/care manager leader to coordinate multidisciplinary patient care across the continuum. You will organize care sequences to maximize effectiveness and outcomes, collaborating with physicians, nurses, case managers, and payers to ensure smooth transitions and high-quality results.

You will demonstrate Service Excellence at all times, with additional duties as assigned.

Qualifications

• Minimum of two (2) years Social Work Experience
• Prior experience in case management or discharge planning
• Educational requirements include High School Diploma/GED and various higher degrees
• Licensure/Certification: Care Management Certification

Responsibilities

Serves as the lead in multidisciplinary planning and coordination of patient care across the healthcare continuum. Organizes the sequence of patient care to attain maximum levels of effectiveness and efficiency to reach the desired quality and outcomes. Utilizing a team approach, this position coordinates and organizes multiple strategies to ensure patients are managed with minimal overlapping roles and processes. Collaborates with physicians, nurses, other case managers, direct and indirect providers, community resources, and payers.
