Job summary

The Aging and Disability Services Division (ADSD) in Elko, Nevada seeks a Social Worker 1 to provide case management and direct support to at-risk elderly and people with disabilities in the Office of Community Living- Intake program.

Under supervision, you will assess needs, develop care plans, coordinate services, conduct home visits, and document activities using a web-based system to promote community integration and prevent institutionalization.

Qualifications

• Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker, or Associate in Social Work.
• Assess and reassess client levels of functioning by interviewing clients and others; evaluate resources to determine needed services.
• Develop and implement case plans including placement, training, socialization, and financial assistance per guidelines.

Responsibilities

• Provide ongoing case management services including home visits, service delivery monitoring, and progress tracking.
• Coordinate with state agencies, community organizations, and private providers to meet client needs and goals.
• Organize MDT meetings to plan protective services and coordinate service delivery.
• Document activities and prepare reports according to policies and regulations.
