Job summary

Aging and Disability Services Division in Nevada seeks a Social Worker 2 in Elko to provide case management and essential services under the Office of Community Living- Intake (OCL). Qualifications

• Requires licensure as a Social Worker or equivalent and 1 year of social work case management experience.
• Ability to assess client functioning via interviews and records, and determine services.

Responsibilities

• Manage and coordinate case plans to meet client needs within regulatory guidelines.
• Perform home visits, assess needs, track progress and adjust plans as needed.
• Record case narratives and prepare reports for law enforcement and courts as required.
• Coordinate resources across state agencies and community providers.
