Job summary

The incumbent coordinates with external agencies, participates in policy development, and provides crisis intervention as needed. Licensure as a Social Worker in Nevada and one year of professional case management experience are required.

Qualifications

• Must hold licensure or provisional licensure as a Social Worker (or clinical/independent/associate in Social Work) in Nevada.
• Minimum one year of professional experience providing case management in a social work setting.
• Experience with evaluating client needs and coordinating services across agencies.

Responsibilities

• Identify mental health disorders and conduct evaluations.
• Engage in case management and coordinate with external agencies for vulnerable adults.
• Develop and implement case plans and monitor progress and service delivery.
• Coordinate MDT meetings and assist in policy/procedure development.
• Provide crisis intervention and report neglect or abuse when required.
• Document case activities and maintain case histories.
• Supervise/lead junior staff or trainees as assigned.
• Perform related duties as assigned and exemplify professional practice.
