Job summary

The State of Nevada, Aging and Disability Services Division (ADSD) in Las Vegas, seeks a full-time Social Worker 2 underfill. This underfill role advances to the next level based on performance and availability.

The position covers HCBS and APS programs, performing assessments, care planning, and community-based services to support vulnerable adults and elders. Responsibilities include eligibility determination, home visits, documentation, and collaboration with families, attorneys, and agencies

Qualifications

• Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker or Associate in Social Work.
• One year of professional experience providing case management in a social work setting (equivalent to Social Worker I in Nevada).

Responsibilities

• Assess and reassess client levels of functioning through interviews and assessments.
• Develop and implement individualized case plans with services per regulatory guidelines.
• Coordinate services across state agencies, community organizations, and private providers.
• Provide ongoing case management including home visits, monitoring and reassessment.
• Organize multidisciplinary team meetings for protective services planning.
• Document case activities and prepare required reports and correspondence.
