Qualifications

• Bachelor's degree from a CSWE-accredited school.

• Hawaii State Social Work license or eligible.
• BLS Certification.
• Minimum 2 years in long-term care/healthcare or elderly care."
• Knowledge of medical terminology.

Responsibilities

• Provision and coordination of counselling services.
• Admission, discharge, and transfer planning and coordination.
• Coordination of abuse prevention programs and investigations.
• Develop discharge plans from acute care to community settings.
