Job summary

Ohana Pacific Health in Honolulu, HI seeks a dedicated Social Worker to manage psychosocial, emotional, environmental, and discharge planning needs for residents. The role involves supporting guests and families from discharge through community transition and leading abuse prevention efforts as part of an interdisciplinary team.

Responsibilities include counselling provision, admissions/discharges/transfers, and developing safe, community-focused care plans, with emphasis on smooth transitions

Qualifications

  • Bachelor's degree from a CSWE-accredited program.
  • MSW/DSW preferred from a CSWE-accredited program.
  • Hawaii State Social Work license or eligibility.
  • BLS Certification.
  • Minimum 2 years in long-term care, healthcare or elder care preferred.
  • Knowledge of medical terminology.

Responsibilities

  • Provide counselling services and coordinate psychosocial support.
  • Arrange admissions, discharges, and transfers.
  • Coordinate abuse prevention program and investigations.
  • Develop plans of care to ensure safe discharge from acute care.

Education

Bachelor's degree in social work
Master's or Doctoral degree in social work preferred

Salary Range: $64,274.47 To $72,844.40 Annually

The Position:

We are looking for a caring and passionate Social Worker to manage the delivery of support services to residents including the identification and assessment of psychosocial, emotional, environmental, spiritual, discharge and support needs. They support our guests, and their families, from the time of discharge, and as they transition back into the community. Our Social Workers also lead abuse prevention programs as an integral part of an interdisciplinary team and are also responsible for:



  • Provision and coordination of counselling services

  • Arrangement for proper admissions, discharges, and transfers

  • Coordination of abuse prevention program and abuse investigations.

  • Provide a smooth transition from an acute care environment, with special consideration on developing a plan of care which will support a safe and appropriate discharge


Requirements:


  • Bachelor's degree from a school of social work accredited by the Council on Social Work Education

  • Master's or Doctoral degree from a school of social work accredited by the Council on Social Work Education preferred

  • Hawaii State Social Work license or eligible.

  • BLS Certification.

  • Minimum of 2 years experience in long term care, healthcare setting or work with the elderly preferred.

  • Knowledge of medical terminology.
