Job summary

East Hawaii Region of Hawaii Health Systems Corporation in Hilo is seeking a Social Worker IV to provide advanced social work and case management across acute, long-term, behavioral health, and outpatient settings at Hilo Benioff Medical Center.

You will coordinate discharge planning, advocate for patients and families, educate on directives and resources, and collaborate with interdisciplinary teams to ensure safe, compliant, patient-centered care.

Qualifications

• MSW with advanced social work experience and supervision potential.
• One year of specialized social work experience in complex case management,Healthcare preferred.
• Valid Hawaii driver's license.

Responsibilities

• Conduct comprehensive psychosocial assessments and provide counseling across care settings.
• Coordinate discharge planning with physicians, teams, and post-acute providers.
• Educate patients/families on advance directives, healthcare decisions, and resources.
• Advocate for patients, coordinate guardianship and referrals as needed.
• Provide guidance to staff, ensure regulatory compliance, and participate in quality initiatives.
• Maintain documentation and participate in care planning.
