Job summary

Viva Health in Birmingham, AL is seeking a Social Worker, Care Manager to support members' social, emotional, and environmental needs as part of care management.

You will develop care plans, coordinate with community resources, and monitor progress, traveling to multiple locations within the service area while maintaining high ethical standards and patient-centered care.

Qualifications

• BSW required.
• Three years in social work.
• Current LBSW license in good standing in the State of Alabama.
• Valid Driver's license in good standing.
• Face-to-face member contact with duties away from principal location.
• Willing to submit to vaccine testing and screening.
• Excellent interview and telephone skills; ability to work with older adults.
• Knowledge of community resources and health-related service delivery systems.
• Strong communication and relationship-building skills.
• Organizational and time-management abilities.
• Flexible, adaptable, and able to work in various settings.
• Basic computer skills.

Responsibilities

• Assess social, emotional, and environmental needs of Care Management members.

The Social Worker, Care Manager is responsible for assessing Care Management members' social, emotional, and environmental needs, developing participants' social plans of care, negotiating of community health services required under the plans of care, and ongoing monitoring and re-assessment of Care Management members. This position will travel to locations within the VIVA HEALTH service area through a reliable means of transportation insured followingCompany policy.

REQUIRED:

• BSW
• Three years in social work
• Current LBSW license in good standing in the State of Alabama
• Valid Driver's license in good standing
• May require significant face-to-face member contact with duties regularly performed away from the principal place of business
• Willing to submit to vaccine testing and screening
• Excellent interview and telephone skills, as well as the ability to deal with older adults in a caring and helpful manner
• Working knowledge of and ability to access community resources
• Working knowledge of health-related service delivery systems
• Excellent communication and relationship skills
• Ability to analyze varied environmental factors concerning members' wellbeing
• Organization and time management skills
• Ability to be flexible, adaptable, and able to work effectively in a variety of settings

PREFERRED:

• MSW
• One year of social work experience in the field of community resource coordination
• Current LMSW license in good standing in the state of Alabama
• Ability to utilize Microsoft Word, Outlook, and Excel
