Job summary

Enriching-Lives-3 in Rancho Cordova, CA is seeking a Social Worker (Coordinator Emphasis) to oversee services for Individuals by developing and implementing the Individual Service Plan, coordinating with the Planning Team, and ensuring regulatory compliance.

You will work with Family Home Providers, support crisis management, and liaise with healthcare professionals, funding sources, and regulators to maintain high quality care.

Qualifications

• Bachelor’s degree plus 1–2 years relevant experience preferred; or equivalent

Responsibilities

• Coordinates the development & implementation of the Individual Service Plan with the Planning Team.
• Ensures provision of medical, health, and ancillary services per the ISP.
• Monitors and supports Family Home Providers during visits and implementation of ISPs.
• Provides crisis management support and rotating on-call coverage.
• Develops and maintains relationships with individuals, families, funding sources and regulatory agencies.
• Liaises with consultants (behaviorists, nurses, therapists, physicians) to meet service needs.
• Performs other duties as assigned.
