Comunidade Metodista in Napa, CA seeks a Social Worker / LCSW to manage a caseload and coordinate care with an IDT. You will conduct psychosocial assessments, develop care plans, provide counseling and case management, and liaise with families and the care team.

The role emphasizes care transitions, discharge planning, and collaboration with physicians to ensure high-quality outcomes for frail populations.

Qualifications

• MSW required and licensed or eligible for licensure.
• Minimum of one year experience with frail or elderly populations.
• Experience coordinating care conference meetings.
• Experience assisting with behavioral health or substance use issues preferred.

Responsibilities

• Conduct psychosocial assessments and participate in IDT care planning.
• Deliver and document social work interventions, including resources and counseling.
• Coordinate smooth care transitions with primary care and care team.
• Initiate and facilitate care conferences to ensure coordinated care.
• Provide discharge planning when participants disenroll from the program.
