Job summary

Oakpointe Senior Care and Rehab Center in Detroit, MI, seeks a Social Worker to provide social services for residents and families, coordinating care planning and referrals to community resources. This is a full-time role aimed at supporting residents through illness and placement decisions.

The ideal candidate may hold a Social Work degree or be licensed, with 1–2 years' supervised health-care experience. Excellent customer service and teamwork are essential to success.

Qualifications

• Degree in Social Work is preferred but not required.
• Licensed in Social Work preferred but not required.
• 1-2 Years of supervised social work experience in a health care setting; nursing home experience preferred.

• Team-oriented and dedicated to supporting teammates.

Responsibilities

• Identify the medically-related social service needs of residents and assist with ensuring those needs are met by the appropriate discipline.
• Maintain contact with family (with resident’s permission) to report changes in behavior and care planning participation.
• Assist with financial and legal matters such as guardianship and pre-paid funeral arrangements.
• Coordinate discharge planning services and community referrals to support home or facility care.
• Assist with assessment and care planning to support residents’ individual needs and preferences.
