Oak Street Health is seeking a Medical Social Worker to support psychosocial needs and coordinate comprehensive care for high-risk patients. You will work with an interdisciplinary team to assess, advocate, and connect patients and caregivers with needed resources to improve health outcomes.

The role emphasizes education on advance care planning, family support, and problem-solving within a dynamic primary-care setting. Full-time, on-site position with competitive benefits.

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages common to the communities served.
• Proficient computer skills (MS Office, GSuite, EMR).
• US work authorization.

Responsabilités

• Proactively assess high-need patients via telephonic check-ins and in-person visits.
• Collaborate with patients and care networks to set goals and execute care plans.
• Refer to community resources to address healthcare and psychosocial needs.
• Navigate insurance benefits with the PRM to maximize plan utilization.
• Provide longitudinal and episodic care coordination to empower self-management.
• Educate patients on advance care planning and goals of care.
