Amberwoods of Farmington is seeking a Social Worker to join our team for Case Management of patients on our short-term rehabilitation unit.

Qualifications

• Bachelor's degree in Human Services or Social Work.
• Nursing Home or case management experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Complete EMR documentation and assessments in a timely manner.
• Complete intake for new admissions including discharge planning.
• Coordinate services to support discharge after short-term rehab.
• Hold Care Plan Conferences with the interdisciplinary team.
• Assist residents and families with concerns.
• Support residents' psychosocial needs.
• Maintain accurate documentation.

Amberwoods is a 130-bed beautifully renovated, successful facility providing long-term skilled nursing, short-term rehabilitation and Memory Care services, located in Farmington, CT. Our facility is conveniently located within proximity to I-84 and is a ten-minute drive to downtown Hartford.

• Completes required EMR documentation in a timely manner including assessments.
• Completes intake for new admissions including discharge plan.
• Uses her/his resources to set up services for successful discharge after short-term rehab.
• Holds Resident Care Plan Conferences with the interdisciplinary team to review resident's plan of care.
• Assists the interdisciplinary team with any concerns that arise with residents and/or families.
• Support residents' psychosocial needs.
• Maintains timely and accurate documentation of above.

Experience

• Nursing Home or case management experience preferred but can train the right person.

Education Requirements

• Bachelors Degree in Human Services or Social Work
