Amberwell Health Atchison is seeking a part-time social worker to provide services across acute care, maternal child care, emergency department, and home health/hospice settings.
You will identify psychosocial needs, develop and implement plans of care, coordinate discharge and crisis intervention, educate patients and families, and connect them with community resources, collaborating with a multidisciplinary care team.

Provide social work services to patients in acute care, maternal child care, emergency department, home health/ hospice setting for patients with acute, chronic, or terminal illness and their families. Services to patient and families include: identification of psychosocial needs, develops, and implements plan of care from the strengths perspective responsive to patient/family concerns, health care status, available resources, age specific problems and clinical impressions; discharge planning, crisis intervention, information and referral, protective services, patient family education, collaboration with the multidisciplinary care team, and building connections with community resources.
