Confidential Skilled Nursing & Long-Term Care Facility near Jenkintown, PA is seeking a Full-Time Social Worker Assistant to join our interdisciplinary team and support residents and families throughout their stay.

The role includes discharge planning coordination, emotional support, community resources navigation, and advocacy for residents’ psychosocial well-being under the Social Worker Director. Excellent communication and compassionate care are essential.

A compassionate and resident-focused Skilled Nursing and Long-Term Care Facility near Jenkintown, PA is seeking a Full-Time Social Worker Assistant to join our interdisciplinary team. This position is ideal for someone who is passionate about supporting residents and their families while helping ensure a positive experience throughout their stay.

The Social Worker Assistant will work closely with the Social Worker Director to provide emotional support, assist with discharge planning, coordinate community resources, and advocate for residents' psychosocial well-being.

Social Worker Assistant Responsibilities

• Knowledge of discharge planning and community resources is a plus.
