NewYork-Presbyterian is seeking a Social Worker for the Adult Sickle Cell outpatient multidisciplinary care team. The role involves biopsychosocial assessments, individualized care planning, and ongoing psychosocial support to patients and families navigating chronic illness.

You will collaborate with physicians, nurses and other team members, and maintain EMR documentation. Preferred candidates have outpatient health/mental health experience and Spanish fluency; LMSW or LCSW licensure is

Qualifications

• Licensed LMSW or LCSW in New York State.
• Strong assessment, counseling and crisis intervention skills.
• Experience with outpatient health/mental health care preferred.
• Spanish language ability preferred.

Responsabilités

• Conduct biopsychosocial assessments and develop individualized plans of care for patients and families.
• Provide counseling, crisis intervention, advocacy and psychoeducation.
• Coordinate care with physicians, nurses and other team members; facilitate family meetings.
• Maintain accurate electronic medical records and communicate with the Sickle Cell team.
