NewYork-Presbyterian Hospital is seeking a Social Worker for the Adult Sickle Cell program, dayshift, Monday–Friday, 9am–5pm at Columbia Irving Medical Center. You will assess biopsychosocial needs, develop care plans, provide counseling, crisis intervention and ongoing support to patients and families navigating chronic illness.

The role requires licensure (LMSW or LCSW) and strong collaboration within an interdisciplinary team.

Qualifications

• New York State LMSW or LCSW license is required.
• Experience in outpatient health/mental health settings preferred.
• Strong assessment, counseling and crisis-intervention skills.
• Ability to collaborate with an interdisciplinary care team and family members.

Responsabilités

• Conduct comprehensive biopsychosocial assessments and develop individualized plans of care for patients and families.
• Provide counseling, crisis intervention, advocacy, psychoeducation, and ongoing psychosocial support.
• Maintain accurate electronic medical records and communicate with the Sickle Cell treatment team.
