Job Description

The Licensed Social Worker (LMSW) provides short-term assistance to older adults through phone consultations, identifying their needs and offering referrals to appropriate services. The position requires coordination with internal programs and external agencies to ensure client access to essential resources. Advocacy, outreach, and follow-up are key components, aimed at helping clients maintain stability and independence.

Responsibilities / Job Duties :

• Conduct phone assessments to identify concerns and urgent needs of older adults
• Provide appropriate internal and external referrals to partner agencies
• Advocate to ensure clients access the services and resources they require
• Collaborate with social service and case management organizations
• Maintain accurate documentation of all client interactions and referrals
• Stay current on available resources and community services
• Participate in training and professional development opportunities
• Empower older adults in navigating support systems

Minimum Education and Experience Qualification Requirements :

• Master’s degree in Social Work (LMSW required)
• Experience working with geriatric populations
• Strong phone communication and interpersonal skills
• Proficiency in Microsoft Word, Excel, Zoom
• Willingness to learn and adapt to internal systems
• Preferred : Experience with Salesforce and bilingual (Spanish) skills
