DESCRIPTION OF WORK

This role centers on providing social services, supporting residents, and coordinating care to meet individual needs. As a Social Worker 2, you will perform the following duties:

• Care Plan Participation: Join interdisciplinary team meetings, complete documentation, and integrate psychosocial assessments with person-centered plans
• Behavioral Support: Provide behavioral assessments, implement appropriate interventions, and offer guidance to staff on behavior management strategies
• Rights and Directives: Review resident rights, assist with advanced directives, investigate grievances, and verify voting status
• Counseling Services: Provide individual and group counseling, offer crisis support, and guide residents and families through emotional challenges
• Discharge Coordination: Organize transfers by addressing needs, coordinating with families, arranging services, and ensuring transportation and follow-up care
• Policy Compliance: Follow safety rules, support quality improvement efforts, use protective equipment, and maintain strong professional relationships

Work Schedule and Additional Information:

• Full-time employment, 37.5 hours per week
• Work hours are 1 shift (8:00 AM to 4:00 PM), Monday - Friday, with a 30-minute lunch.
• Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment); or
• Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor's degree in social work or social welfare; or
• Two years of professional experience providing direct public or private social work services, and a bachelor's degree in social work or social welfare; or
• A master's degree in social work or social welfare.
• Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Special Requirements:

• You must possess an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
