Qualifications

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) with continuous commonwealth employment since Sept 8, 2008; or two years as Social Worker 1/Forensic Social Worker 1 with equivalent Federal title and a bachelor’s degree in social work or social welfare.
  • Two years of professional experience providing direct social work services, with a bachelor’s degree in social work or social welfare; or a master’s degree in social work or social welfare.
  • Applicants will be considered to meet educational requirements if within 3 months of graduating with a qualifying degree.

Responsibilities

  • Care Plan Participation: join interdisciplinary meetings, complete documentation, and integrate psychosocial assessments with person-centered plans.
  • Behavioral Support: provide assessments, implement interventions, and advise staff on behavior management strategies.
  • Rights and Directives: review resident rights, assist with advanced directives, investigate grievances, verify voting status.
  • Counseling Services: provide individual and group counseling, crisis support, and guide residents and families through emotional challenges.
  • Discharge Coordination: organize transfers, address needs, coordinate services, arrange transportation and follow-up care.
  • Policy Compliance: follow safety rules, support quality improvement, use PPE, and maintain professional relationships.

DESCRIPTION OF WORK

Work Schedule and Additional Information
  • Full-time employment, 37.5 hours per week
  • Work hours are 1 shift (8:00 AM to 4:00 PM), Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.

QUALIFICATIONS
REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
Minimum Experience and Training Requirements
  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment); or
  • Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor’s degree in social work or social welfare; or
  • Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare; or
  • A master's degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
Special Requirements
  • You must possess an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.

Legal Requirements
  • A conditional offer of employment will require a medical examination .
  • This position falls under the provisions of the Older Adult Protective Services Act.
  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
