Qualifications

• Degree in Social Work or related field; Bachelor's preferred
• Minimum 2 years of social work experience in a long-term care setting
• Experience using PointClickCare or similar health information management system; proficient with MS Word, Excel and Outlook
• Excellent interpersonal and listening skills; patient/resident focused; able to set priorities, solve problems and manage conflict

Responsibilities

• Assess residents' emotional, social, and psychological needs and develop individualized care plans

Social Worker Job Summary:

We are seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in ensuring the emotional and social well-being of our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, provide counseling and support services, and facilitate communication between residents, families, and healthcare providers.

Social Worker Responsibilities:

• Assess the emotional, social, and psychological needs of residents and develop individualized care plans to meet those needs

• Provide counseling and support services to residents and their families, addressing issues related to adjustment, coping, and loss

• Facilitate communication between residents, families, and healthcare providers to ensure that residents' needs are met and concerns are addressed

• Coordinate with other members of the healthcare team to ensure that residents receive comprehensive and coordinated care

• Develop and implement strategies to promote socialization and community building among residents

• Maintain accurate and up-to-date records of resident assessments, care plans, and progress

• Participate in quality improvement initiatives to improve the overall quality of care and services

Social Worker Requirements:

• Degree in Social Work or related field required, Bachelor's preferred

• 2 years of experience in social work in a long term care environment

• Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
