Qualifications

• Degree in Social Work or related field; Bachelor's preferred
• Minimum 2 years of social work experience in a long-term care setting
• Experience using PointClickCare or similar health information management system; proficient with MS Word, Excel and Outlook
• Excellent interpersonal and listening skills; patient/resident focused; able to set priorities, solve problems and manage conflict

Responsibilities

• Assess residents' emotional, social, and psychological needs and develop individualized care plans
• Provide counseling and support to residents and families on adjustment, coping, and loss
• Facilitate communication between residents, families, and healthcare providers
• Coordinate with the healthcare team to ensure comprehensive, coordinated care
• Develop and implement strategies to promote socialization and community building among residents
• Maintain accurate records of resident assessments, care plans, and progress
• Participate in quality improvement initiatives to improve care and services
