Skilled Nursing Care in New Jersey is seeking a full-time Social Worker to support residents in a well established nursing home. You will evaluate patients, develop care plans, and coordinate with the care team to meet individual goals.

The role requires SNF/LTC experience, a bachelor’s degree (master’s preferred), and at least 3 years in healthcare social work. You will communicate effectively with residents, families, and regulatory staff to ensure high-quality care.

Qualifications

• Must be able to work full-time hours and be flexible.
• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum three (1) years’ experience as a social worker in a healthcare-related setting, nursing home or rehabilitation center preferred.
• Knowledge of rules, regulations, and guidelines governing nursing care facilities.
• Excellent communication skills with residents, families, and staff.

• Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager

• Build rapport with patients and their families

• Coordinate care with PT departments

• Develop care plans for patients

• Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
