Plaza Health, a well established nursing home, is seeking an experienced Social Worker to join our compassionate team in New Jersey. You will evaluate patients, address goals with patients and families, coordinate care with PT, and develop care plans.

A Bachelor's degree is required; a Master's is strongly preferred, and SNF/LTC experience is essential. We offer health, vision and dental benefits, a 401(k) plan, and a competitive salary ranging from $70,000 to $90,000 per year, commensurate with

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum three years’ experience as a social worker in healthcare.
• Experience in nursing home or rehabilitation setting preferred.
• Knowledge of nursing home rules, regulations, and guidelines.
• Excellent communication with residents, families, staff, and agencies.

Responsibilities of Social Worker:

• Evaluate patients (existing, new and prospective) based on their needs and limitations

• Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager

• Build rapport with patients and their families

• Coordinate care with PT departments

• Develop care plans for patients

Qualifications of Social Worker:

• Must be able to work full-time hours and be flexible

• Bachelor’s degree required. Master’s degree is strongly preferred.

• Minimum three (1) years’ experience as a social worker in a healthcare-related setting. Experience within a nursing home or rehabilitation center is strongly preferred.

• Knowledge of the rules, regulations, and guidelines that govern nursing care facilities.

• Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public.
