Job summary

Waters Edge Health and Rehab is seeking a Social Service Assistant to join our social services department at our Wisconsin facility. You will work closely with residents to ensure quality care and support social and emotional needs.

This full-time role offers Monday–Friday day hours and opportunities to collaborate in interdisciplinary care planning, discharge planning and resident education, with a focus on dignity and individualized resident goals.

Qualifications

• Experience in long term care or SNF.
• Experience working with a diverse aging population.
• Social work setting experience is preferred.

Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
