Job summary

Sharon Center for Health & Rehabilitation is hiring a Social Worker to advocate for residents' well-being and ensure holistic needs are met. You will collaborate with families and the care team to create individualized plans and provide crisis support as needed.

The role requires a Master’s in Social Work, active licensure, and prior long-term care experience, with strong counseling and communication skills. The team emphasizes compassionate, person-centered care.

Qualifications

  • Master’s degree in social work from an accredited program.
  • Current licensure or certification as a Social Worker in the state of employment.
  • Prior long-term care/geriatrics experience preferred.
  • Strong assessment, counseling, and crisis intervention skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work in a multidisciplinary team.

Responsibilities

  • Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences
  • Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns
  • Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes
  • Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes
  • Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs
  • Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status

What You\'ll Do:

Rate of Pay

USD $30.00 - USD $32.00 /Hr.


What You\'ll Bring:

Qualifications of a Social Worker:

Master’s degree in social work from an accredited program


Current licensure or certification as a Social Worker in the state of employment


Previous experience in Long-Term Care, geriatrics, or a related field


Strong assessment, counseling, and crisis intervention skills


Excellent communication and interpersonal abilities


Ability to work collaboratively as part of a multidisciplinary team


Empathy, compassion, and a commitment to person-centered care
