Job summary

Bloomfield Center for Nursing & Rehabilitation in Bloomfield, CT is seeking a dedicated Social Worker to advocate for residents and coordinate psychosocial care. You will assess needs, develop care plans with the interdisciplinary team, and provide counseling to residents and families.

Qualifications

• Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes.
• Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes.
• Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs.
• Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status.

What You'll Do:

Rate of Pay

USD $35.00 - USD $45.00 /Hr.

What You'll Bring:

Qualifications of a Social Worker:

Master’s degree in social work from an accredited program

Current licensure or certification as a Social Worker in the state of employment

Previous experience in Long-Term Care, geriatrics, or a related field

Strong assessment, counseling, and crisis intervention skills

Excellent communication and interpersonal abilities

Ability to work collaboratively as part of a multidisciplinary team
