Job summary

National Health Care Associates, Inc. Sharon Center for Health & Rehabilitation seeks a dedicated Social Worker to advocate for residents’ well-being and coordinate holistic care.

You will assess psychosocial needs and collaborate with families and the interdisciplinary team to support residents throughout care transitions. Responsibilities include conducting psychosocial assessments, counseling, advocacy for resident rights, and connecting families with community resources.

Qualifications

• Master's degree in social work from an accredited program.
• Current licensure or certification as a Social Worker in the hiring state.
• Experience in Long-Term Care, geriatrics, or related field preferred.

Responsibilities

What You'll Do:

As a Social Worker, you will advocate for the well-being of our residents and ensure their holistic needs are met. Your work will play a pivotal role in enhancing the quality of life of our residents.

Key Responsibilities of a Social Worker:

Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences

Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns

Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes

Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes

Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs

Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status

Rate of Pay

USD $30.00 - USD $32.00 /Hr.

What You'll Bring:

Qualifications of a Social Worker:

Master's degree in social work from an accredited program

Current licensure or certification as a Social Worker in the state of employment

Previous experience in Long-Term Care, geriatrics, or a related field

Strong assessment, counseling, and crisis intervention skills

Excellent communication and interpersonal abilities

Ability to work collaboratively as part of a multidisciplinary team

Empathy, compassion, and a commitment to person-centered care
