Job summary

Love Joy Adult Day Health Care Center is seeking a dedicated and compassionate Social Worker to enhance the well-being of our clients in Sacramento, California. In this full-time position, you will conduct assessments, develop personalized care plans, and provide essential counseling support.

The ideal candidate holds a Master's degree in Social Work or a Bachelor's with relevant experience. Strong communication skills and knowledge of community resources are crucial, along with CPR and First Aid certifications. Bilingual skills (Vietnamese, Korean) are preferred.

About the Role

Responsibilities

• Conduct assessments to determine clients’ social, emotional, and psychological needs.
• Develop and implement personalized care plans in collaboration with clients and their families.
• Provide counseling and support to clients and their families to navigate challenges.
• Coordinate with healthcare providers to ensure holistic care delivery.
• Facilitate support groups and educational workshops for clients and caregivers.
• Maintain accurate and confidential client records in compliance with regulations.
• Advocate for clients’ rights and access to community resources.
• Stay updated on best practices and trends in social work and elder care.

Requirements

• Master's degree in Social Work (MSW) or other related field
  • or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting
• Strong communication and interpersonal skills with a compassionate demeanor.
• Ability to work collaboratively in a multidisciplinary team environment.
• Knowledge of community resources and services for seniors.
• Proficient in documentation and familiarity with electronic health records.
• Commitment to ongoing professional development and ethical practice.

Additional Requirements

Current CPR and First Aid certification

Bilingual skills are strongly preferred (Vietnamese, Korean)
