Qualifications

• Bachelor of Science in Social Work or related field.
• Minimum 2 years of experience as a Social Worker.
• LSW/LCSW/LICSW preferred but not required, license may be advantageous.

Responsibilities

• Assist residents and families in connecting with community resources and services.
• Conduct assessments to identify psychological, financial, religious, and physical needs.

• Assists residents and their families in connecting with community resources, healthcare providers, and other social services
• Conducts assessments and evaluations to identify psychological, financial, religious, and physical needs

• Collaborates with other disciplines to ensure comprehensive care and support
• Provides counseling and support services to residents and their families
• Maintains accurate and detailed records of resident interactions and services provided

Requirements:

• Education: Bachelor's of Science in Social Work or a related field
• Experience: Minimum 2 years of experience as a Social Worker

• Skills: Strong communication, assessment, and social work methods and techniques; ability to work effectively with a variety of disciplines; knowledge of social systems and ability to implement pertinent state and federal regulations
