AdviniaCare Providence seeks a compassionate Social Worker to join our long-term care team in Providence, RI. You will help residents and families access community resources, assess needs, and design personalized care plans to support well-being.

The role requires strong communication, documentation, and collaboration with various disciplines to ensure holistic care and adherence to relevant regulations.

Qualifications

• Bachelor of Science in Social Work or related field.
• Minimum 2 years of experience as a Social Worker.
• LSW/LCSW/LICSW preferred but not required, license may be advantageous.

Responsibilities

• Assist residents and families in connecting with community resources and services.
• Conduct assessments to identify psychological, financial, religious, and physical needs.
• Develop and implement individualized care plans.
• Collaborate with other disciplines to ensure comprehensive care.
• Provide counseling and support to residents and families.
• Maintain accurate records of interactions and services.
