Job summary

AdviniaCare Salem is seeking a dedicated Licensed Social Worker (LSW, LCSW, or LICSW) to serve as Director of Social Services. You will lead the social services program, assess residents at admission, and coordinate with nursing, therapy, and administrative teams to ensure holistic, person-centered care for our seniors.

You will maintain ongoing communication with families, assist our Admissions Coordinator with welcoming tours, and support transitions into the facility.

Qualifications

• Licensed social worker (LSW, LCSW, or LICSW) with degree from an accredited school of social work.
• Minimum 2+ years of experience in long-term care.
• Passion for seniors, teamwork, problem-solving, and compassionate care.

Responsibilities

• Ensure holistic, person-centered resident care.
• Lead social, psychological, and social support efforts; coordinate with nursing, therapy, and administrative teams.
• Complete social assessments at admission and pre-admission assessments; gather information.
• Maintain ongoing communication with residents' families and assist Admissions Coordinator with welcoming tours.
• Support resident transitions into the facility.

• Complete social assessments and gather important information at admission
• Maintain ongoing communication and support with residents' families
• Assist our Admissions Coordinator by giving thoughtful, welcoming tours to prospective residents and families
• Complete pre-admission assessments to help residents transition smoothly into our facility

What You Bring:

• A degree from an accredited school of social work
• Licensure in the state of practice (LSW, LCSW, or LICSW required)
• 2+ years of experience in long-term care preferred
• A heart for seniors and a passion for teamwork, problem-solving, and compassionate care
